Non-union Fracture Treatment

Evaluation and treatment of fractures that have failed to progress to healing.

A non-union is a fracture-healing problem in which the bone fails to progress toward healing within the expected period and requires further evaluation.

Contributing Factors & Evaluation

Possible contributing factors include:

  • - Inadequate mechanical stability
  • - Poor blood supply
  • - Infection
  • - Bone loss
  • - Poor bone biology
  • - Smoking
  • - Nutritional or metabolic factors
  • - Certain medications or medical conditions

Evaluation Protocol

Assessment may include:

  • Clinical examination
  • Serial X-rays
  • CT scans when required
  • Blood tests
  • Evaluation for infection
  • Assessment of bone alignment and mechanical stability
  • Review of nutritional, metabolic, and other patient factors

Types of Non-Union

Hypertrophic non-union

The bone may demonstrate biological healing activity, but mechanical stability may be inadequate.

Atrophic or oligotrophic non-union

There may be limited biological healing activity, poor blood supply, bone loss, or other biological factors.

Infected non-union

Infection may be present at the fracture site and requires specific evaluation and treatment.

Treatment Options

Treatment is directed at the underlying cause and may include:

  • • Revision fixation
  • • Improving mechanical stability
  • • Bone grafting
  • • Treatment of infection
  • • Correction of deformity
  • • Biological augmentation in selected cases
